Purpose: The overarching aim of this training series is to improve care of older Veterans. This webinar series presents latest advances and findings on topics related to aging/geriatrics based on innovative research, education and clinical activities of VA GRECCs (Geriatric Research, Education and Clinical Centers). The target audience is frontline VA staff that provide services to older Veterans. This program is aligned with the VA Secretary’s Priority regarding Customer Service/Care, and VA’s Strategic Plan goals (Goal 2) emphasizing that Veterans receive highly reliable and integrated care and support and excellent customer service. This webinar will specifically address appropriate prescribing in the emergency department. Suboptimal prescribing for older patients has been described in a number of Emergency Department (ED) based and community-based studies. EQUiPPED is a quality improvement initiative aimed at changing prescribing practices of Emergency Department (ED) providers to decrease the use of potentially inappropriate medications (PIMs), as defined by the Beers Criteria (updated in 2019), for older adults discharged from the ED. EQUiPPED was developed by an interdisciplinary team using the Vision-Analyze-Team-Aim-Measure-Map-Change-Sustain (VA-TAMMCS) model of process improvement, which was developed within VA from Lean and Six Sigma principles. The EQUiPPED team in now in its 8th year as a collaborative and the program is implemented in 20 VA EDs nationally. Multiple sites have demonstrated a significant and sustained reduction of ED-prescribed PIMs in older Veterans after implementation of EQUiPPED.
Learning Objectives:
At the conclusion of this learning activity, participants will be able to:
1. Identify conditions that increase the risk of adverse drug events in older adults
2. Determine resources for promoting medication safety in the ED setting
3. Describe strategies that promote successful implementation of an ED-focused quality improvement program targeting medication safety.
